1.  Send us an email with your full name and phone number.

2.    Complete the Residency Application Form and send to us at:  Riverside Physical Therapy, 1619 NW Hawthorne Ave., Ste 109, Grants Pass, OR 97526.

3.  You will need to provide us with 3 Letters of Recommendations.

4.  Provide the Recommendation Form to your Recommender(s) to complete and to fax back to us at:  (541) 476-2397.

5.    You will need to submit a non-refundable Application Fee of $25.

6.     You will also need to submit a Commitment Holding Fee of $750 which will be refunded back to you after 6 months in the residency program.  If you have not been accepted into the residency program, you will receive the full refund within 10 business days.

7.  We will then contact you to set up an interview.

8.     A Resident Handbook will be issued to you at the start of the residency program.
